Please include FULL NAMES of both parents that would have been on the baptismal record of Candidate.
- Please note, if parent is deceased, divorced, etc. we still need need their name listed the sacramental record (assuming they were listed on child's baptismal record.)
- Do not use nicknames (Jim for James, etc.) 
- Include middle name
- Include Mother's maiden name
